Short answer. NexLife plans renew automatically until cancelled, and cancellation requires 30 days' notice during which you are still billed. Medication is non-refundable once your clinician has ordered it. The advertised guarantee refunds program fees, not medication, after 180 days and only if you meet every condition; NexLife's own example puts the refundable amount at about $250 of $834 paid over six months.
NexLife advertises compounded tirzepatide from $139 a month with 12 months paid up front, $169 for a single month, and a weight-loss guarantee. The guarantee is the part most patients remember. The cancellation policy is the part that decides what you actually pay if you leave, and it is more specific than the headline.
Disclosure first: I founded SkinnyVIP, a compounded tirzepatide program with different terms, and I compare them at the end. Everything attributed to NexLife below is quoted from its own published policy pages on October 18, 2026, with links, and the quotes are chosen to answer the questions patients actually ask: when is money refundable, what renews, and how do you stop it. Nothing here is a judgment of NexLife's clinical care, which I have no way to assess.
Automatic renewal
“Automatic Renewal: Your payment method will be automatically charged at regular intervals, as outlined during the checkout process, until you cancel your subscription. The first renewal may be charged early to ensure continuity of service. Subsequent renewals may be charged and shipped up to three (3) days early to accommodate holidays or other operational needs.” — NexLife, Cancellation Policy
Two details matter here. The renewal can be charged early, and the interval is whatever was shown at checkout, so the checkout screen is the document to keep.
The 30-day notice
“You may cancel your subscription at any time by emailing care@nexlife.us or calling (949) 818-8000.” — NexLife, Cancellation Policy
“Clients must provide a 30-day notice prior to their desired cancellation date. This 30-day notice may be waived in exceptional circumstances, such as severe medical conditions, subject to approval by Nexlife's medical team.” — NexLife, Cancellation Policy
“Unless waived due to exceptional circumstances, the effective date of cancellation will be 30 days from the date the cancellation notice is received. Clients will continue to receive services and be billed accordingly until the effective cancellation date.” — NexLife, Cancellation Policy
In practice: the day you email is not the day billing stops. If a renewal falls inside the 30 days after your notice, the policy as written allows it to be charged. The way to avoid an unwanted cycle is to send notice more than 30 days before the renewal date shown at checkout, and to keep the email.
What is refundable
“No Refunds for Shipped Medications: Any medication that has already been dispensed, shipped, or received by the patient is non-refundable.” — NexLife, Cancellation Policy
“Unfortunately, we cannot cancel or refund medication shipments after they have been ordered by your provider.” — NexLife, Terms & Conditions
“There will be no refunds of any subscription fees paid prior to the cancellation becoming effective.” — NexLife, Terms & Conditions
“For one-time products and services, if your medical questionnaire has been reviewed and approved, and your prescription has been sent to the pharmacy, the amount paid is non-refundable.” — NexLife, Cancellation Policy
The cutoff is the clinician's order to the pharmacy, which is earlier than shipping. There is a prorated-refund clause, but it applies only when NexLife cannot fulfil refills “due to regulatory restrictions, supply shortages, or unforeseen circumstances,” and refunds for partially used periods are “not typically offered but may be provided on a case-by-case basis at Nexlife's discretion” (Cancellation Policy).
How the guarantee refund actually works
The guarantee page is clear that it covers fees, not medication:
“Program and membership fees are refundable. Medication costs are not. Medication is dispensed by an independent licensed pharmacy and cannot be returned, so its cost is excluded from any refund under this policy.” — NexLife, Refund Policy (guarantee)
“Because medication makes up most of what you pay, the refundable amount will be substantially less than your total spend.” — NexLife, Refund Policy (guarantee)
“For a member on the $139/month plan, approximately $250 of the $834 paid over six months is refundable.” — NexLife, Refund Policy (guarantee)
Eligibility, per the same page: enrolled on or after January 1, 2026; an active member for at least 180 consecutive days; medication taken as prescribed without refill gaps; weight logged at least every two weeks in the portal; less than 5% of starting body weight lost; starting BMI of 30 or above; and the request emailed between day 181 and day 210. The terms page adds a video weigh-in requirement and states that “The final eligibility decision rests solely with Nexlife and is non-negotiable” (Terms & Conditions). None of this makes the guarantee meaningless; it makes it a refund of roughly 30% of six months' spend, available to a patient who meets every condition. That is the number to weigh.

|---|---|---|
| Renewal | "Automatically charged at regular intervals... until you cancel." Renewals "may be charged and shipped up to three (3) days early." | Nothing renews. Before you switch any program: four things to confirm
- Where the refund cutoff sits. "Routed to the pharmacy," "dispensed," and "shipped" are three different moments, sometimes days apart. Know which one your program uses.
- Whether the next charge is already scheduled. If a plan renews, find the renewal date and the cancellation deadline in writing before you do anything else.
- What a cancellation actually costs. A notice period, a re-rating of discounted months, or a non-refundable consultation fee can all apply. Read the sentence that says so.
- What continuity looks like. Ask any new program how quickly a licensed clinician reviews your intake and when the first shipment arrives, so there is no gap in doses.
Frequently asked questions
Does NexLife have a cancellation fee or notice period?
NexLife's cancellation policy requires a 30-day notice before the desired cancellation date, and states that clients continue to receive services and be billed until the effective date. The notice may be waived in exceptional circumstances subject to approval by NexLife's medical team. No separate cancellation fee is stated.
Can I get a refund from NexLife once my medication is ordered?
Not under the published terms. NexLife states it cannot cancel or refund medication shipments after they have been ordered by your provider, and that medication dispensed, shipped or received is non-refundable. A prorated refund clause applies only when NexLife cannot fulfil refills due to regulatory restrictions, supply shortages or unforeseen circumstances.
How much does NexLife's weight-loss guarantee refund?
Program and membership fees only, not medication. NexLife's own example: on the $139/month plan, approximately $250 of the $834 paid over six months is refundable, and only if every eligibility condition is met, including 180 consecutive active days, bi-weekly weight logging, a starting BMI of 30 or above, and a request sent between day 181 and day 210.
Does NexLife renew automatically?
Yes. The cancellation policy states that your payment method will be automatically charged at regular intervals until you cancel, and that renewals may be charged and shipped up to three days early.
A program where nothing renews.
$695 for a 3-month plan (any dose, about $232 a month) or $350 for a single month. Refundable until the pharmacy dispenses. Full refund of the medication cost if a licensed clinician does not prescribe. A real person answers within one hour, 7 days a week, 9am to 9pm ET.
