Short answer. Affinity Direct’s tirzepatide page says “From $120.14/mo,” which it defines as the 2.5 mg price on “the 3-month plan with optional auto-refill (Subscribe & Save, 15% off, cancel anytime),” “billed $360.40 per 3-month shipment.” Each dose has its own price, from $120.14 to $162.64 per 4 weeks, and the page itself notes that an Affinity “month” is “one 4-week (28-day) supply.” A one-time month starts at $174. No membership fee. The terms say auto-refill “will automatically renew” unless you cancel “before the next renewal date,” that “Cancellation takes effect at the end of the current billing period,” that “No refunds are issued for the current period upon your cancellation,” that you are “refunded in full” if a clinician does not approve, and that dispensed medication is not refundable. No pharmacy is named.

Affinity Direct publishes a full price for every dose, a one-time option, and its own explanation that its “month” is four weeks. That is more arithmetic than most programs put on a page, and it means the headline figure needs translating before you compare it to a calendar-month price. The terms, last updated September 10, 2026, carry the renewal and refund rules. This article puts those sentences next to the marketing.

Disclosure first: I founded SkinnyVIP, a compounded tirzepatide program with different terms, and I compare them at the end. Everything attributed to Affinity Direct below is quoted from its own published pages on October 19, 2026, with links, and the quotes are chosen to answer the questions patients actually ask: when is money refundable, what renews, and how do you stop it. Nothing here is a judgment of Affinity Direct's clinical care, which I have no way to assess.

What the product page says

“Starting at $120.14/mo · all-in — medication, licensed provider review, injection supplies and cold shipping, with no membership fee — effective on the 3-month plan with optional auto-refill (Subscribe & Save, 15% off, cancel anytime).” — Affinity Direct, compounded tirzepatide page, read October 19, 2026
“Billed $360.40 per 3-month shipment (a 12-week supply), not monthly.” — Affinity Direct, compounded tirzepatide page
“Or $174 one-time for a 1-month supply with no subscription.” — Affinity Direct, compounded tirzepatide page
“2.5 mg $120.14/mo $174 one-time 5 mg $128.64/mo $209 one-time 7.5 mg $137.14/mo $224 one-time 10 mg $145.64/mo $239 one-time 12.5 mg $154.14/mo $254 one-time 15 mg $162.64/mo $269 one-time” — Affinity Direct, compounded tirzepatide page
“An Affinity tirzepatide "month" is one 4-week (28-day) supply of weekly doses, and auto-refill renews one supply at a time; lower and higher are judged per calendar month, a 4-week price counting 13 times a year.” — Affinity Direct, compounded tirzepatide page
“Affinity tirzepatide starts at $120.14 per 4 weeks ($130.15 per calendar month), effective on the 3-month plan with auto-refill — Subscribe & Save, 15% off, cancel anytime.” — Affinity Direct, compounded tirzepatide page
“New patients typically start at the lowest dose.” — Affinity Direct, compounded tirzepatide page

Three translations are needed. First, $120.14 is the 2.5 mg price with auto-refill on; the one-time price at that dose is $174. Second, the price moves with dose: $162.64 per 4 weeks at 15 mg with auto-refill, or $269 one-time. Third, a “month” is 28 days, so there are 13 of them in a year; the page gives its own calendar-month equivalent, $130.15, for the starting figure. The $360.40 is one 3-month shipment at 2.5 mg, charged at once and again at each renewal if auto-refill is on.

What the terms say about renewal

“If you select the Subscribe & Save option, you are enrolling in an automatically renewing subscription.” — Affinity Direct, Terms of Service, last updated September 10, 2026
“Your subscription will automatically renew at the end of each billing period (monthly or as selected) and your payment method on file will be charged at the then-current subscription price, unless you cancel before the renewal date.” — Affinity Direct, Terms of Service
“For subscriptions, we will notify you of any change to your recurring price before it takes effect, and the new price will apply to renewals after the effective date unless you cancel.” — Affinity Direct, Terms of Service
“Subscribe & Save auto-refill (15% off) is optional, starts only if you authorize it separately at checkout, and can be paused or cancelled anytime from your patient portal.” — Affinity Direct, compounded tirzepatide page
“Every plan can be bought as a one-time order, which ends after delivery with no future charges.” — Affinity Direct, compounded tirzepatide page

Auto-refill is a choice at checkout, and the 15% discount comes with it. If you take the discount, the renewal is automatic at the then-current price, with notice of changes. The one-time price ends after delivery. Decide which you are buying before you look at the headline figure.

How to stop, and when it’s too late

“You may cancel, pause, or modify your subscription at any time by: (1) logging into your patient portal and adjusting your subscription settings, or (2) contacting our support team at virtual@affinitywholehealth.com before the next renewal date.” — Affinity Direct, Terms of Service
“Cancellation takes effect at the end of the current billing period.” — Affinity Direct, Terms of Service
“No refunds are issued for the current period upon your cancellation.” — Affinity Direct, Terms of Service

There is no notice period in days; the deadline is the renewal date itself. Cancelling stops the next charge, not the current one, and the current period is not refunded. On a 3-month plan that period is 12 weeks, so cancel before the renewal rather than after it.

Refunds

“If a provider does not approve treatment, your payment is refunded in full (please allow 5–10 business days for the refund to appear, depending on your bank).” — Affinity Direct, Terms of Service
“Prescription medications are not eligible for a refund once dispensed.” — Affinity Direct, Terms of Service
“Where we have charged you for treatment that has not been dispensed, we will refund that amount; amounts for treatment already dispensed are governed by the Refund Policy in Section 7.” — Affinity Direct, Terms of Service
“Delivery delays do not qualify for a refund.” — Affinity Direct, Terms of Service

A full refund if a clinician does not approve; a refund of anything charged but not yet dispensed; nothing once dispensed; and no refund for the current period when you cancel an auto-refill. The cutoff is “dispensed,” and the terms say a lost or damaged shipment is handled by reshipment rather than refund.

The pharmacy

“Prescriptions are dispensed and shipped by licensed pharmacies that exercise their own professional and regulatory responsibilities.” — Affinity Direct, Terms of Service
“Your medication ships from a licensed U.S. pharmacy in cold-chain packaging with discreet labeling, and injection supplies are included.” — Affinity Direct, compounded tirzepatide page

No pharmacy is named on the pages we read. The name of the pharmacy and the source of its active ingredient are reasonable questions to ask support before the first order.

Before you switch any program: four things to confirm

  1. Where the refund cutoff sits. "Routed to the pharmacy," "dispensed," and "shipped" are three different moments, sometimes days apart. Know which one your program uses.
  2. Whether the next charge is already scheduled. If a plan renews, find the renewal date and the cancellation deadline in writing before you do anything else.
  3. What a cancellation actually costs. A notice period, a re-rating of discounted months, or a non-refundable consultation fee can all apply. Read the sentence that says so.
  4. What continuity looks like. Ask any new program how quickly a licensed clinician reviews your intake and when the first shipment arrives, so there is no gap in doses.
